A&S Shipping Form

For any shipment, please fill out the form provided below. Provide as much detail as possible and complete all sections as this will expedite the shipping process and limit further questions. Once the form is submitted you will receive a confirmation email. In order to guarantee same-day shipment of your item, please have completed the below form no later than 2pm. The package also must be ready to ship and brought to the JSB dock shipping/receiving location by this time as well. Please note that some shipments may require further paperwork or packing materials and this may delay the shipping process.  

Note: the process above is for UPS services only as they are our preferred carrier.  Under special circumstances when FedEx must be used (eg. 3rd party payment, etc), same-day shipping cannot be guaranteed due to FedEx polices and procedures.  FedEx services cannot be used for DOT/IATA hazardous material shipping due to regulatory agreements/contracts with the university.

*Shipping paperwork and printed labels can only be generated by A&S shipping staff if the package is physically located here at the university; any shipments originating from an external location must be completed by that external location's personnel due to regulatory requirements.*
